What does nurse-led care consist of?
Nurse led care provides dedicated support for your illness from specialist trained nurses with the knowledge and experience to provide expert nursing care at home. Your nurse will monitor your health and record medical history and symptoms, so its progress is reported and available for any doctor check-ups.
They can manage and administer medications and treatments and educate you and your family on the management of your illness as well as providing support and advice. They can also liaise with other health and social care professionals involved in your care to provide a joined-up approach to your care and health outcomes.
