What is post-hospital care?
Post-hospital care is about providing the right kind of support to keep you safe and independent when you leave hospital. A trained carer will visit you at home and step in to help where needed. This might mean doing chores, assisting with medication or helping with personal care – whatever you need to get settled in and feel yourself again.
Your carer is also there to make the transition from hospital to home that little bit easier. They know that hospital treatment can be physically and emotionally draining – and returning home after a long stay in hospital can be stressful. That’s why they’ll always be patient, understanding and ready to join you for a cuppa and a chat.
Post-hospital care is sometimes known as ‘after-hospital care’, ‘intermediate care’ or ‘aftercare’.

Our post-hospital care packages are highly flexible. Whether you need 24/7 live-in care or occasional visits to help with housework and errands, our friendly carers are ready to support you. Our carers are also DBS-checked and are trained to support people with a wide variety of health conditions, including mobility problems, dementia and spinal injuries.

Live-in post-hospital care
With live-in care, you get 24-hour support from a dedicated carer. They’ll live with you on a long-term basis, so you’ll always have help at hand when you need it. Live-in care might be a good option if you have mobility difficulties or a complex condition that needs specialised support. It also means extra reassurance for you and your family – especially if you live alone.
Visiting home care after hospital discharge
Some people can cope quite well after hospital but need a little bit of support from time to time. That’s where visiting care comes in. With this option, a carer visits you as often as you need to help you get back on your feet. For instance, they might pop in each morning to help you get dressed or visit once a week to deliver groceries. We’ll create a tailored care schedule built around your unique needs and the things you want to achieve.
Respite care after hospital discharge
Do you need temporary home care after surgery or illness? Our respite care service might be a good fit. It’s a type of short-term live-in care designed to help you settle in and get on the road to recovery. It’s ideal if you need extra support during those difficult first few weeks after leaving hospital. It’s also a good way to try out live-in care if you’re still deciding whether it’s right for you.
Overnight after-hospital home care
Overnight care is ideal for people who have support in the daytime but need care or companionship during the night. You might choose this option if you have special care needs during the early hours, for instance, or simply feel more anxious at night. For you and your family, it means extra reassurance that help is always at hand.
Companionship care
Going back to an empty house after a stay in hospital can be daunting. Our companionship carers are there to make settling in at home that little bit easier. They can pop in from time to time for a cuppa and a catch-up. Or they can stay with you around the clock to ease loneliness and help with housework. It’s all about giving you the support you need to feel safe, happy and comfortable when you most need it.

Respite care is a type of short-term care. You might choose respite care after hospital discharge if you know you’ll only need support for a little while. It’s also a good option if you’re not sure whether live-in care is right for you but want to try it out.
What is post-operative care at home?
Post-operative care at home is a specialised form of care for those who need health care or home help after surgery. It’s completely tailored to the individual but often includes personal care (washing and dressing), mobility support and assistance with medical devices like catheters.
Can you provide temporary home care after surgery?
Yes. Choose our respite care service and we’ll provide tailored care at home for as long as you need it. This could be for as little as a few days or as much as several months.
Do you get free care when you come out of the hospital?
Some people are eligible for up to six weeks of free care and support at home after leaving hospital. However, this isn’t yet available in all parts of the UK. If free post-hospital home care is available in your area, hospital staff can help arrange it for you.
What happens when an elderly person leaves the hospital?
Before an elderly person leaves hospital, they will be assessed by hospital staff to make sure they’re ready to go home. If staff believe the person is fit enough to be discharged but needs extra support at home, they should arrange this on the person’s behalf. You can also arrange care yourself by contacting a care provider like Novus Care.
What can I do if I have no one to take care of me after surgery?

Sleeping night care
Choose sleeping night care and an expert carer will sleep in your home between 10pm and 7pm. If you need support, your sleep-in carer will be happy to get out of bed to assist you. This option is best for those who might need help once or twice at night and want the reassurance that someone’s there to provide it.
Waking night care
Need someone around who’s ready to help at a moment’s notice? With this option, a waking night carer visits your home between 10pm and 7am but doesn’t go to sleep. Waking night care might suit you if you need specialised care throughout the night or want extra peace of mind in case there’s an emergency.
Overnight respite care
Do you have a regular carer who needs a break or wants to take night times off? Our overnight respite care service can help. We’ll send a highly trained carer to cover these breaks so you have the 24/7 support you need. This might be for a few nights, a few weeks or even just a few hours if you need emergency cover.
